In consideration for being allowed to participate and engage in SLBF events and activities, I agree to the terms and conditions (“Terms”) stated below.
1. Assumption of Risk
I understand that participation in activities, programs, and events at Upper House, Dottie’s Ranch or any other hosting facility may involve certain risks, including but not limited to physical activity, transportation to and from the facility, and use of facility and the surrounding area, including, but not limited to,  being in close proximity to other participants, eating food provided or prepared by SLBF staff and/or volunteers and participating in walking or hiking excursions. I acknowledge that these risks could result in injury, illness, and/or property damage or loss and that I have a responsibility for my own safety, protection and comfort.
2. Voluntary Participation
I certify that my participation is voluntary and that I assume all risks associated with my involvement in any events or activities organized by SLBF or that take place at any SLBF facility or any other hosting facility (the “Activities”).
3. Release of Liability
In consideration of being allowed to participate, I hereby release, waive, and discharge SLBF, its directors, officers, employees, volunteers, agents and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may occur during my participation in the Activities.
4. Indemnification
I agree to indemnify, defend, and hold harmless SLBF, its directors, officers, employees, volunteers, agents and affiliates from and against any and all claims, demands, damages, losses, liabilities, costs, and expenses (including reasonable attorney’s fees) (collectively, “Claims”) arising out of or related to my participation in the Activities, including but not limited to any Claims brought by third parties as a result of my actions or omissions.
5. Medical Authorization
In the event of an emergency, I authorize SLBF employees, or their on-site delegates, to secure and/or provide medical treatment to me that is deemed necessary. I understand that I am responsible for any medical expenses incurred.
6. Photography and Media Release
I hereby grant SBLF, its designees and successors, my consent and permission to use my name, photograph, image, likeness, voice and/or biography and to take, use, reproduce, edit, and distribute photographs, images, likenesses, video recordings, or other media of me taken during the Activities (the “Media”). This includes SLBF’s use of the Media in promotional materials, websites, social media, publications, and any other lawful purpose, without compensation. I waive any right to inspect or approve the finished product or any use of such Media and release SLBF, its directors, officers, employees, volunteers, agents and affiliates from any claims related to the use of the Media, including claims of invasion of privacy, defamation, or rights of publicity. 
7. Miscellaneous
These Terms shall be governed by and construed under the laws of the state of Wisconsin.  I agree that in the event any clause or provision of these Terms is deemed invalid, the enforceability of the remaining provisions shall not be affected.
